The Department of Social Services at Affinity realizes the cost
of prescription drugs can be a huge financial strain on patients.
To ensure patients have access to their medications and can obtain
them at affordable prices, the department provides patient assistance
programs, which are offered through various pharmaceutical companies.
These programs are designed to help patients obtain at least four
of their most expensive medications--significantly decreasing their
prescription cost for the period they are approved to receive assistance.
To determine if they are eligible for these programs, patients
must complete the necessary applications--providing their medical,
physician and financial information. (Each program has different
guidelines.) There is a $40 charge for a three-month supply of
name-brand medications. Re-application is usually required every
three to six months, depending on the pharmaceutical company. In

Be sure to bring the following materials to your appointment:
- All medications you are taking
- A printout from your pharmacy
- Proof of household income, e.g.,
documentation from Social Security of the amount of your check
each month, pay stubs from your employer, or last year's federal
taxes (form 1040)
Affinity's Department of Social Services has two social workers
and an LPN on staff. The social workers interview patients and
order their medications through the patient assistance programs.
They see approximately 1,000 patients on a quarterly basis (four
times a year). The department also provides nursing home placement
and pre-certification for tests and procedures at Affinity.
Q. Who qualifies for these patient
assistance programs?
A. You must be
under the care of an Affinity physician and must not have prescription
coverage of any kind. Income guidelines vary for each company.
Q. How long does it take once I
apply to obtain my medications?
A. The
time frame varies with each company. Typically, it takes four to
six weeks for your medications to arrive.
Q. How often will I need to be seen
to renew my medications?
A. Medications
are usually sent in three-month increments. Therefore, you will
need to be seen every three months
to order refills.
Q. What if my medicines change?
A. If there are any changes in your
medicines, have your doctor notify our office, and we will make the
necessary arrangements.
Q. Can I obtain my medications from
any physician I choose?
A. Affinity
Health Group can only be liable for medicines prescribed by an
Affinity physician.
Q. What if I don't qualify?
A. Generally
our social workers can inform you whether or not you will be approved
at the time of your appointment.
Q. How long can I participate in
the program?
A. There is no limit as to how long
you can receive medicines through the program as long as you continue
to qualify.
